How do I approach my doctor about depression?

I am fairly sure I have depression. I’m failing my classes because I don’t care to do homework or go to class. I go for days without sleep, and then sleep for 20+ hours straight. My eating habits are all off. I have no hope for the future, and have been cutting for almost 10 years. I know I need help, but how do I bring this up? I have been too shy/nervous to do so before, and haven’t told anyone how I feel. I just don’t know how to break this silence.
Any advice on how to bring up the subject?
